PCHP hosts several Community Link Workers that work across North West Edinburgh. Link Workers work in local GP practices, meeting people that have been referred by GPs or other practice staff to provide one to one support for a short period of time.
Link Worker referrals might be made for a wide range of reasons, such as benefits issues, housing problems, loneliness or mental health conditions. Over several sessions, Link Workers aim to link people with services, community groups and activities which could help to improve their wellbeing.
A Link Worker’s support can enable people to set goals and overcome barriers so that they can take greater control of their health and wellbeing. By using ‘good conversations’ the Link Workers support people to identify problems and issues they are experiencing and to talk about what really matters to them. They support people to achieve their goals by helping them to identify and access relevant resources or services in their community.
The Link Workers also map local services and keep their GP practice teams informed of existing and new services and identify any local service gaps.
PCHP currently employs 6 Community Link Workers, in joint partnership with Edinburgh Health and Social Care Partnership funded by the Scottish Government. Our CLWs are currently based in 7 local practices accepting referrals from clinical staff for registered patients over 16.

You cannot self-refer to the Link Worker service, but if you ask your GP, they may be able to refer you on.
