Job Title:          GP Community Link Workers (3 full time posts available)

Working hours: 35 hrs per week
   
Rate of pay:  £24,700 per annum plus 6% contribution to pension scheme

Holidays:  Annual leave entitlement: 25 days, 10 public holidays and 2 days     floating (Total – 37 days).

Contract:  Fixed term contract until the end of 31st March 2019 (it is      anticipated further funding would be available beyond this date)

Interviews:             Monday the 21st of August or Tuesday 22nd of August 2017    

As a result of new Scottish Government Funding we are seeking to recruit an additional 3 proactive and dynamic Community Link Workers to join our existing team at this exciting pilot.  The aim is to work within our GP Link Worker team to develop excellent working relationships with GP practices; providing them with a community link service which takes into account the individual demographics and circumstances of each practice. Link workers will provide 1:1 person-centred support to people referred by GP’s, whilst enabling people to identify personal outcomes and priorities for their health and well-being.
